When a person is very near death, the need to relieve pain may become more important than managing the breathing rate. At this time, narcotics (opioids) may be used to keep a person comfortable, even at doses that may cause respiratory depression.
It is important to remember that the dying process itself leads to changes in breathing that may not be related at all to narcotics.
