Tools for Creating Your Own Advance Directive

By using the interactive tools found here, you can examine your own ideas about living with advanced illness and use those ideas to:

create a medical directive ("living will")
assign a person to be your health care advocate

The documents you create, print, sign and have witnessed will help guide your family and your health care providers in following your wishes about medical treatment at a time when you can no longer communicate. You can also create a checklist to make sure all your documents are in order and can be found easily.
